Community Care Services Program (CCSP)

Community Care Services 
Program Overview

At Zeppone Caring Hands, we proudly participate in the Community Care Services Program (CCSP) to help seniors and adults with physical disabilities remain in their homes and communities, rather than move into institutional care settings. CCSP is a Medicaid home- and community-based waiver program designed to provide essential support services to eligible Georgians who meet nursing home level of care but prefer to live independently.

Administered by the Georgia Department of Community Health (DCH) through local Area Agencies on Aging (AAAs), CCSP connects clients with comprehensive health, personal, and social services tailored to their individual needs.

Services We Provide Through CCSP

Our care team collaborates with clients, families, and physicians to create a custom care plan and deliver the following Medicaid-approved services:

  • Adult Day Health
    Medically supervised group care including activities and therapies such as physical, occupational, or speech therapy.

  • Alternative Living Services
    24-hour care and supervision in licensed personal care homes.

  • Emergency Response Services
    A 24/7 in-home communication system for immediate help during emergencies.

  • Home-Delivered Meals
    Nutritious, balanced meals delivered directly to the client’s home.

  • Home-Delivered Services
    Skilled nursing and therapeutic services delivered at home following home health discharge.

  • Out-of-Home Respite Care
    Short-term relief for primary caregivers, available at personal care homes or adult day centers.

  • Personal Support Services
    Help with daily living activities like personal care, meal preparation, housekeeping, shopping, and in-home respite care.

Who is Eligible?

To qualify for CCSP, individuals must:

  • Have a functional impairment due to physical limitations

  • Be approved by a physician for an intermediate level of nursing home care

  • Be Medicaid eligible or potentially eligible based on financial screening

  • Choose home- and community-based care over nursing facility care

  • Participate in only one waiver program at a time
