SOURCE Program Overview

At Zeppone Caring Hands, we proudly participate in the SOURCE (Service Options Using Resources in a Community Environment) program, designed to connect eligible individuals with vital medical and long-term care services in the comfort of their own homes or community settings.

The SOURCE program integrates primary medical care and case management with a wide range of home and community-based services. It serves individuals who are eligible for full Medicaid and who meet the Intermediate Nursing Home Level of Care, supporting them in maintaining independence and dignity while receiving high-quality care.

Operating under the Georgia Elderly and Disabled 1915(c) Medicaid Waiver, SOURCE is tailored to meet the unique needs of frail elderly and physically disabled individuals. As of 2016, over 18,000 participants were enrolled in this impactful program.

How It Works

  • A comprehensive assessment is conducted to determine each individual's eligibility and level of need.

  • Based on the assessment, a personalized care path is created, focusing on medical oversight and assistance with daily living tasks.

  • The care plan is developed collaboratively—involving family members, caregivers, support agencies, and medical providers.

  • Participants benefit from coordinated services designed to enhance their health, safety, and quality of life.

  • Home-Delivered Meals

  • Emergency Response Systems

  • 24/7 Medical Access to a case manager and primary care physician
