Independent Care Waiver 
Program Overview

At Zeppone Caring Hands, we are dedicated to helping individuals with physical disabilities and traumatic brain injuries (TBI) maintain their independence and live safely in the comfort of their own homes or within the community. Through the Independent Care Waiver Program (ICWP), eligible participants receive personalized, Medicaid-funded services that serve as an alternative to nursing home or hospital care.
 

ICWP is a Home- and Community-Based Waiver (1915c) authorized by the Centers for Medicare and Medicaid Services (CMS). The program is designed to support adults who are medically stable but at risk of institutionalization due to the absence of accessible community care.

Who is Eligible?

To qualify for ICWP, individuals must:

  • Be between 21 and 64 years old at the time of application

  • Be Medicaid eligible

  • Have a severe physical disability or traumatic brain injury (TBI)

  • Require assistance with one or more activities of daily living (ADLs)

  • Be medically stable yet at risk of placement in a hospital or nursing facility

  • Be able to live safely in a home or community setting

  • Be capable of directing their own care (TBI applicants are exempt from this requirement)

  • Additional eligibility considerations include current residence (e.g., hospital or nursing facility), time spent on the waiting list, and the cost-effectiveness of providing care at home versus institutional care.

Services Offered Under ICWP

Based on an individual’s assessed needs, services may include:

  • Service Coordination

  • Personal Support Services

  • Home Health Care

  • Emergency Response Systems

  • Respite Care

  • Counseling Services

  • Specialized Medical Equipment & Supplies

  • Home Modifications

Each participant works closely with their case manager and family to develop a customized plan of care that outlines their needs, goals, available services, and budget. Approval is contingent on funding and state Medicaid agency review.
