Our dedicated team collaborates closely with Hospice Care providers to ensure that individuals can spend their remaining time in the comfort of their home or familiar surroundings, surrounded by loved ones. We offer support services at home in Savannah and surrounding areas.

Alzheimer’s and other forms of dementia present significant emotional and physical challenges. Hospice care becomes particularly beneficial during the end stages of these illnesses. ZCH supports dementia patients by managing symptoms and enhancing quality of life, thereby reducing anxiety for both patients and their families.

When is the right time for Hospice?
Hospice is a treatment designed for patients who have a life expectancy of six months or less. Six months or less can be difficult to access with dementia due to the slow decline of many patients. Only a doctor can order a hospice evaluation or make a clinical determination of life expectancy. However, here are common signs that Alzheimer’s or dementia has progressed to a point where a patient will likely benefit from hospice care:

How can Hospice help myself or a loved one?
Patients who utilize hospice services report a higher quality of life than those who don’t. Hospice patients can take comfort in knowing that they are not alone in their treatment. Patients receive care based on their specific health issues, and they specify what services they receive and don’t receive.
